DETECTION OF MACRO-THROTROPIN IN PATIENTS WITH HASHIMOTOS THYROIDITIS AND SUBCLINICAL HYPOTHYROIDISM

    Аннотация The level of thyroid stimulating hormone (TSH) is one of the diagnostic markers of thyroid function. In subclinical hypothyroidism, the concentration of TSH in the blood serum increases, while the level of thyroid hormones stay the same. The reason for this observation is the macroTSH phenomenon, in which the macro isoforms of TSH (a complex of TSH with IgG) are present in the blood. Blood serum collected from 50 patients (30 patients with subclinical hypothyroidism, 10 - with manifest hypothyroidism, 10 - control group) was screened for the presence of macroTSH by polyethylene glycol (PEG)-precipitation and analyzed using gel filtration chromatography. As a result of the screening, 56–98% of 50–100% serum TSH was identified as true macroTSH. In patients with subclinical hypothyroidism with the TSH level of more than 10 μIU/ml, a trend towards an increase in the level of macroTSH has been shown. It is known that elevated levels of antibodies to thyroperoxidase (TPO) can lead to the generation of macroTSH. Thus, we identified that the amount of macroTSH complex in patients with subclinical hypothyroidism, in whom the level of antibodies to TPO is >500 U/L, is significantly higher compared with patients with manifest hypothyroidism. Patients with the TSH level of more than 10 μIU/ml are candidates for screening for the presence of the macroTSH complex.
